Compounded tirzepatide: regulatory state, concentrations, what to verify

Compounded tirzepatide has spent two years inside an active legal fight between Eli Lilly and US compounding pharmacies. What's available, what's defensible, and what to verify before you reconstitute.

Verified May 10, 2026: Compounded tirzepatide has lived through a more turbulent legal environment than compounded semaglutide. Eli Lilly removed Mounjaro and Zepbound from the FDA drug shortage list in late 2024, which narrowed the window in which 503A pharmacies could legally compound the same molecule. What ships in 2026 sits in a smaller, more carefully sourced market than two years ago.

How compounded tirzepatide differs from Mounjaro and Zepbound

The active molecule — tirzepatide, a dual GIP/GLP-1 receptor agonist — is the same across compounded and manufactured versions. Practical differences show up here:

  • Concentration varies more than semaglutide. A 30 mg vial reconstituted in 1 mL gives 30 mg/mL. The same vial in 2 mL gives 15 mg/mL. The same vial in 3 mL gives 10 mg/mL. Three different syringe-unit answers for the same prescription.
  • Vial sizes are not standardized. Pharmacies commonly ship 10 mg, 20 mg, 30 mg, and 60 mg vials. Some ship 100 mg.
  • Storage windows are shorter. Most compounded tirzepatide carries a 28-day BUD post-reconstitution under refrigeration. Some pharmacies extend this to 42 or 56 days; verify in writing rather than assuming.
  • Additives are less common. Compared to compounded semaglutide, tirzepatide preparations less often include B-vitamin adjuncts.

The titration ladder still applies

Compounded tirzepatide follows the same FDA-labeled ramp as Mounjaro and Zepbound: 2.5 mg → 5 mg → 7.5 mg → 10 mg → 12.5 mg → 15 mg, weekly, with at least four weeks at each step before advancing. Tirzepatide's side-effect profile is steeper step-for-step than semaglutide's. The clinical trials saw all-cause discontinuation rise from ~1.4% at 2.5 mg to ~7% at 5 mg (SURMOUNT-1, NEJM 2022). The 4-week minimum hold is built around giving the dual receptor agonism time to plateau before the next bump.

Questions to ask the pharmacy

Same six as for compounded semaglutide, with an additional emphasis on concentration:

  • Vial mg and BAC water volume (with reconstitution instructions in writing)
  • Additives and concentrations
  • BUD post-reconstitution
  • Pre-reconstitution storage spec
  • 503A or 503B facility
  • API source and certificate of analysis on request

Reconstitution math, where things go wrong

Worked example: a 30 mg vial reconstituted in 2 mL gives 15 mg/mL. A 5 mg target dose works out to 0.33 mL, or 33 units on a U-100 syringe.

The same 30 mg vial in 1 mL gives 30 mg/mL. The same 5 mg dose is now 0.166 mL, or 17 units. Drawing 33 units from this concentration delivers nearly 10 mg — twice the intended dose.

Storage notes

Refrigerate, never freeze. Avoid leaving the vial out for more than a few hours during reconstitution or dosing. Light exposure accelerates degradation more aggressively for tirzepatide than semaglutide; an opaque storage container helps if your fridge has a glass door.

Is compounded tirzepatide as effective as Mounjaro?

The molecule is identical. Effectiveness depends on dosing accuracy and whether the formulation matches the FDA-labeled titration ladder. Reputable 503A facilities follow the same ramp.

Why is compounded tirzepatide more expensive than compounded semaglutide?
